§ Ind. Code § 3-8-7-25.5Statement designating former and current legal name of candidate

(a) This section does not apply to the change of a candidate's name that occurs
after absentee ballots have been printed bearing the candidate's name.
(b) A candidate who:
(1) is:
(A) nominated for election; or
(B) a candidate for nomination; and
(2) changed the candidate's legal name after:
(A) the candidate has been nominated; or
(B) the candidate has become a candidate for nomination;
