§ Ind. Code § 3-6-4.5-8Information required in complaint

The complaint filed under section 7 of this chapter must state the following:
(1) The name and mailing address of the person alleged to be committing the violation
of Title III described in the complaint.
(2) Whether the person filing the complaint has filed a complaint concerning the
violation with a county election board under IC 3-6-5.1.
(3) The nature of the injury suffered (or about to be suffered) by the person filing the
complaint.
