4 A notification under paragraph 1 or a report under paragraph 2 must contain a declaration, made by an individual authorised to do so by the unincorporated association concerned, that to the best of the individual's knowledge and belief— a everything stated in the notification or report is accurate, and b the notification or report contains everything that it is required to contain by this Schedule.
