Particulars in referendum petition. 45. —A referendum petition shall be signed and dated by the petitioner and shall specify— ( a ) the provisional referendum certificate to which it relates, ( b ) the grounds on which it is based, ( c ) the remedy it seeks, and ( d ) the name and address of the petitioner and the petitioner's solicitor or agent, if any.
